  9. NOTICE TO THE PERSON WHO SIGNS THIS WILL:

  12. 1. THIS FORM CREATES A TRUST FOR YOUR DESCENDANTS.IF YOU DO NOT
  13. WANT TO CREATE A TRUST, DO NOT USE THIS FORM.
  14. 2. IT MAY BE IN YOUR BEST INTEREST TO CONSULT WITH A CALIFORNIA
  15. LAWYER BECAUSE THIS STATUTORY WILL HAS SERIOUS LEGAL EFFECTS ON
  16. YOUR FAMILY AND PROPERTY.
  17. 3. THIS WILL DOES NOT DISPOSE OF PROPERTY WHICH PASSES ON YOUR
  18. DEATH TO ANY PERSON BY OPERATION OF LAW OR BY ANY CONTRACT. FOR
  19. EXAMPLE, THE WILL DOES NOT DISPOSE OF JOINT TENANCY ASSETS OR YOUR
  20. SPOUSE'S SHARE OF COMMUNITY PROPERTY, AND IT WILL NOT NORMALLY
  21. APPLY TO PROCEEDS OF LIFE INSURANCE ON YOUR LIFE OR YOUR RETIRE-
